All students must have the
medication form completed before the school can administer medication to
the student. Non-prescription medication forms are to be completed and
signed by the parent/guardian. Prescription medication forms are to be
completed and signed by the physician and parent/guardian.
- No medication will be given to students without the properly signed
forms, no matter how important the medication is to the student.

Do not send medication with your child and instruct them
to take their medication on their own (this would violate the Sumner
County Drug Policy). All medications must be brought to the office.

Forms
will not carry over from one school year to the next. A new medication
must be completed and signed each school year.

A new
form must accompany any changes in medication with these changes noted.
The physician must sign forms indicating changes in prescription
medication.

All
prescription medication must be sent to the school in the pharmacy labeled
bottled with the student’s name and instructions. This bottle must match
the signed physician orders. (Most pharmacies will give you an extra
bottle for the school.)

The
package directions will be followed when giving non-prescription
medication unless accompanied with a physician’s order. All
non-prescription medications must be received in an unopened bottle with
the student’s name on it.

A.M.
(morning) medications need to be given at home at least 30 minutes to one
hour before the start of school.

Antibiotics that are to be given 3 times a day will not be given at
school. (3 times a day means every 8 hours)

Inhalers can be carried with the student as long as the medication form is
signed and on file in the office and the student knows how to properly use
the inhaler.

Any
medication remaining at the end of the school year should be picked up
within two weeks or it will be disposed of appropriately. If this
presents a problem please notify the school to make other arrangements.

The
state of Tennessee may change medication guidelines that the school system
must follow. Please be aware we must follow the guidelines set by
the state.
Please make sure all medical conditions, including allergies, are noted on
the student’s registration card along with emergency contact numbers and
names of adults who are allowed to pick your child up from school.
